New Patient Registration
To register you must reside in the Practice catchment area.
We request all new patients to have a health check with the Health Care Assistant. We cannot fully register you with the Practice until this has been carried out.
In order to register with us you need to:
Step 1: Complete Registration Form
The registration form asks you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the Primary Care Trust to quickly trace your medical records and send them to your new surgery.
Please Note, The new patient registration form is a legal document and needs to be
- filled in
- printed off,
- signed
- and brought to the practice
The form is not electronically sent back to the practice, it is merely completed on screen. Without a correctly completed and signed form the practice can not proceed to register you as a patient.
You must also show us 2 original documents providing proof of identity and address. Click here for details of the documents we can accept.
Complete and print off Registration form
Step 2: Placed on the Practice list
Bring the registration form, and evidence of identity and address to the surgery, so that you can be registered.
Step 3: New Patient Assessment
You will be asked to attend the surgery for a health assessment.
If you do not attend this appointment with the practice nurse/health care assistant we cannot register you at this practice.
